·       Original birth certificate

·       Shot record

·       Address of school they attended

·       One (1) proof of address (power, water, gas)

·       NO phone, cable, insurance or credit card statements. NO EXCEPTIONS!

·       The parent's name must match the name on the bills

·       Parent photo ID

·       Copy IEP if Applicable
